Agency for Persons with Disabilities - Suncoast Region
Program manages the application process and waitlist for Medicaid Waiver. Once approved, services include adult day training, behavioral analysis, medical supplies and equipment, and respite care.
Community Care for Disabled Adults Program provides in-home services for permanently disabled adults ages 18-59 who need assistance to remain in their homes. Agency for Persons with Disabilities Program supports persons with developmental disabilities in living, learning, and working in their community.

Provides safe, affordable housing, supportive housing, and assisted living to individuals age 18 or older with disabilities, including mental health, addiction, and co-occurring disorders.
Easter Seals Southwest Florida
Provides a variety of program services for disabled adults, children, and their families. Some of their programs include respite care to families and caregivers, an adult training program, a children’s therapy center, and preschool for children birth to 5 years old.

Supported employment program offers individuals with a mental health diagnosis, direct job placement, and ongoing support services.
Epilepsy Services of Southwest Florida
Provides low-income and uninsured clients with the resources necessary to empower them to self-management to live an independent life as much as possible. Services provided to at-risk patients include medical, prescription, and case management at little to no cost. Additionally, they offer referrals to other community services and prevention and educational programs to the community.

Foundation for Dreams
Dream Oaks Camp provides a fun, educational, and recreational experience in an outdoor camp environment to enhance the lives of children ages 7-17 with physical and developmental disabilities and serious illnesses. Provides weekend and overnight camps. Camps are week-long in the summer and held on the weekends throughout the year.

Serving those who are blind or have visual impairments. Programs include independent living adult programs, children’s early intervention programs, job readiness, vocational rehabilitation, and support groups. All services are free, and volunteer drivers provide transportation to and from classes.

By being legally appointed as a guardian on these individuals, they can assist and monitor their needs on an ongoing basis, including food, clothing, shelter, and medical conditions.

Barrier Removal Program provides low-income families with a disabled or elderly resident up to a $15,000.00 interest-free loan for the cost of eligible improvements to help disabled residents live independently in their homes.
SHINE (Serving Health Insurance Needs of Elders)
Provides materials and free, unbiased insurance counseling to Florida elders, adults with disabilities, caregivers, and family members. The goal is to help you make informed decisions about health care coverage, Medicare, Medicaid, prescription assistance, long-term care planning, and other issues.

Suncoast Center for Independent Living
Support, guidance, and advocacy for individuals with disabilities. Their programs include applying for benefits, referral, peer mentoring, independent living skills training, support groups, transition assistance, housing, and vocational consultation.
Transportation for the Disabled
Transportation is mandated by the Americans with Disabilities Act for the medically disabled. Riders are screened for eligibility and certification by filling out the application online.
